Request a sick note for work

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website
If you’ve been off sick for more than 7 days
To request a sick note:
- phone the surgery between 10am to 6pm
- Please request a telephone consultation with a GP and give a brief reason why you require a sick note.
